The applicant must have:
- A Curriculum Weighted Average (CWA) of 2.00
- Completed at least 30 units during the application period
The applicant must have:
- Duly accomplished Application form
- Three (3) copies of passport size recent photos
- Application Letter
- True Copy of Grades
- Certificate of Good Moral Character
- Certificate of Non-Contract
- Parent’s Consent

The applicant must have:
- A General Weighted Average (GWA) of 1.75
- Completed at least 30 units
He/she must submit the same requirements identified above for shiftees.
